how to claim medical reimbursement bill documents editable
Directorate of Medical Education, Government of Telangana,will
scrutinize the medical bills. the following original documents should be submitted along with the medical bills Xerox copies will not be accepted, Government Employees, Pensioners are requested to show their Employee /Pensioners ID record at the hospital at the time of
admission and treatment.
DOCUMENTS REQUIRED
i)Forwarding Letter (to be prepared by Head of the Institution )
ii) Application Requisition of individual With date ( to be applied by the employee to the Head of the Institution)
iii) Check list it should be attested by the Forwarding Officer
iv) Appendix –II it should be signed by the Employee or Pensioner and attested by the Forwarding Officer
v) Non –Drawl Certificate
vi) Dependent Certificate with Forwarding Officer signature
vii) Pension Payment Order (PPO) copy in case of pensioner/family pensioner.It is attested by the Forwarding Officer.
viii) FIR should be submitted In case of accident.
ix) For every follow up treatment, re validation of prescription should be given by the doctor for post operative cases and who requires lifelong treatments, once in six month .
x) in case of death of the Teachers , legal Heir Certificate should be submitted by the Pensioners, Dependents. It should be attested by the Forwarding Authority
xi) A copy of recognition of the hospital, issued by the Director of Medical Education should be submitted.
xii) Emergency Certificate, signed and stamped by the treating doctor and it should be attested by Forwarding Officer.
xiv) Discharge Summery and Discharge Memo,given by the doctor.
xv) All the original bills should be signed by the treating doctor and abstract of the bills should be submitted.
xvi) In case of claims where relaxation of rules is involved,the employee should submit the details of proposals along with justification.
